What is Direct Primary Care (DPC)?
Direct Primary Care is a model where patients pay a flat monthly or yearly membership fee directly to the physician for comprehensive primary care services. This model removes insurance billing from routine care, allowing for more personalized, and accessible healthcare.
How do I get started?
Once you have picked a plan, you can click on the join the practice button and sign up to schedule an appointment with the doctor. If you wish, you can schedule a free Meet and Greet with your doctor. Bring your questions regarding Evergreen Direct Primary Care services and see what is the right care plan for you.
Do I need insurance to become a patient at Evergreen DPC?
We prefer that you carry insurance. High-deductible plans work well with our DPC model.
We are NOT insurance and it is advised to have insurance to cover below:
- Labs, radiology and imaging, i.e x-rays, mammograms, MRI, CT scan and ultrasounds (labs and imaging centers will submit the bill to your insurance).
- Medications, vaccinations etc.
- Surgeries, ER visits, urgent care visits, specialist visits
What is not included in your membership?
Only listed services are covered under membership. If you need bloodwork or imaging, we’ll provide an order to complete it at an outside facility. You may use your insurance for these outside services or pay out of pocket. We’re happy to guide you through the process.
Please note: specialist visits, urgent care, and hospital services are NOT included in your membership. Our goal is to keep you healthy and reduce the need for outside care.
Do you take Medicare?
We cannot enroll patients who have any form of Medicare at this time. Please join our waitlist and we will contact you once we open for Medicare patients.
What benefits of DPC membership can I get through my employer?
Some employers are now offering Direct Primary Care (DPC) memberships as part of their benefits package. This means you may be able to access personalized, comprehensive primary care at no additional cost or at a reduced monthly fee through your workplace. Check with your HR or benefits team to see if your company offers a DPC membership—you might already have access! You can ask your employer to contact us to explore the benefits of having DPC for you or your team.
Is DPC more expensive than traditional care?
For many patients, DPC is actually more affordable because:
- There are no copays for office visits
- Fewer unnecessary tests or specialist referrals
- Transparent monthly fees with no surprise bills
Membership is typically a flat monthly fee, billed automatically. No insurance claims are filed for routine care.
Is membership a long-term commitment?
No, there is no long-term commitment. After the first 3 months, you can cancel at any time with a 30-day notice. If you cancel before 3 months, you will be charged a penalty of $300.
Can I use HSA funds to pay for my Evergreen DPC membership?
Yes. You may be able to use funds from your Health Savings Account (HSA) to pay for your Evergreen DPC membership. Please check with your HSA provider or Human Resources (HR) department for information about eligibility and applicable requirements.
